Recognise deterioration early and escalate concerns

By the end of the course, delegates will:

  • Understand what RESTORE2 Mini is and how it relates to the full RESTORE2 tool
  • Understand the triad of clinical outcomes and why RESTORE2 Mini is used
  • Recognise who is at risk of deterioration and its common causes
  • Identify the signs of deterioration and apply the Chain of Safety
  • Know how and how often to record and monitor observations
  • Apply the graded response strategy for at-risk residents
  • Know the questions and checklist used to detect deterioration early
  • Recognise when a situation is a life-threatening emergency
  • Understand what SBARD is and when to use it
  • Apply the SBARD framework to structure an escalation call

Understanding RESTORE2 Mini

  • What is RESTORE2 Mini? — a physical deterioration and escalation tool for care/nursing homes
  • A stepping stone to the full RESTORE2 tool — for homes not yet measuring vital signs
  • The triad of clinical outcomes — right care, right time, right place, right outcome
  • Extra considerations for residents with a learning disability

Recognising deterioration

  • Who is at risk — co-existing disease, acute illness, altered consciousness
  • Common causes — sepsis, infections, chronic disease, unavoidable complications
  • Signs of deterioration — reduced swallow, breathing changes, skin colour, agitation
  • The Chain of Safety — measure, recognise, escalate, respond, document

Observations & graded response

  • Recording and monitoring physiological observations
